Health Worker of the Year Nomination Form

Shining a light across our community. This award honours the health professional who goes the extra mile to provide exceptional care, support, and humanity in every interaction.

Please use this form to nominate someone/organisation for a Pride of Reading Award. Please provide as much information as possible, nominations will be judged on the information provided on the online form. Nominations should be no more than 250 words and the closing date for nominations is Friday 31 July 2026.

All nominations will be reviewed and those shortlisted invited to the Pride of Reading Awards Lunch and Presentations.

We’d like to reassure you that it’s not about how many times someone is nominated – our independent panel of judges focus on the quality and detail within each nomination. A thoughtful submission really does help tell their story.

Nomination tip – please include specific examples of how the health professional has gone above and beyond in their care. Focus on the difference they’ve made to patients, families or colleagues, and highlight moments that show their compassion, dedication and impact in the community.
